Informed Consents

Below are several of our informed consents that we obtain prior to performing a procedure. Depending on what your interests may be, you may wish to review the consent for the procedure you are interested in. It will describe the most common risks and benefits of the procedure. It is not meant to answer all of your questions, but it may give you more insight into the procedure and generate some question that you haven't thought of before.
